We are sorry that your membership request was not successful.  It may be because you canceled the PayPal request or because there was an issue that caused the transaction to not go through.  Whatever the reason, we would like to resolve any questions that you may have concerning joining HSA.  You may also choose to download the form on the How To Join page and mail it in to us if that would work better for you.
